Abstract Introduction The prone position is frequently adopted for surgical or critically ill patients in intensive care. Cardiorespiratory arrest in these patients may pose an operational challenge, in which delays resulting from mobilization to the supine position culminate in worse outcomes. Objective To provide clinical insight based on the synthesis of evidence from reports or case series on reverse cardiopulmonary resuscitation (reverse CPR) in surgical patients or invasive ventilatory support in severe acute respiratory distress syndrome. Methods This is a systematic review of reports or case series in PubMed, Scopus, Embase, and Google Scholar databases, in addition to a search of the gray literature. Case reports published in any language, reporting at least one case of prone cardiopulmonary resuscitation in patients of any age and in any care context, were considered eligible. Results Thirteen studies of fourteen cases of successful reverse resuscitation were retrieved. Three patients died within 30 days, while the others survived without complications or neurological sequelae. Conclusion Despite limited evidence to support clinical decision-making, prone resuscitation appears to be a feasible alternative in exceptional circumstances, where patient mobilization may result in additional harm, delay or interrupt advanced life support (compressions, high-quality early chest surgery, and defibrillation) or incur occupational risks to the health team.


Resumo Introdução A posição prona é um procedimento frequente de cuidados intensivos para pacientes cirúrgicos ou doentes graves. A ocorrência de parada cardiorrespiratória nestes pacientes pode representar um desafio operacional, no qual atrasos relacionados à mobilização para decúbito dorsal implicam em piores desfechos. Objetivo Oferecer um insight clínico a partir da síntese das evidências oriundas de relatos ou séries de casos sobre a utilização de reanimação cardiopulmonar reversa em pacientes cirúrgicos ou em suporte ventilatório invasivo na síndrome do desconforto respiratório agudo grave. Métodos Trata-se de uma revisão sistemática de relatos ou séries de casos condu-zida nas bases de dados PubMed, Scopus, Embase e Google Scholar, além de busca na literatura cinzenta. Foram considerados elegíveis relatos de caso publicados em qualquer idioma, que reportaram pelo menos um caso de reanimação cardiopulmonar em posição prona em pacientes de qualquer idade e em qualquer contexto de atendimento. Resultados Foram recuperados treze estudos que relataram quatorze casos de reanimação reversa bem-sucedidos. Três pacientes faleceram em um intervalo de 30 dias, enquanto os demais sobreviveram sem complicações ou sequelas neurológicas. Conclusão Apesar de evidências limitadas para suportar a tomada de decisão clínica, a reanimação em posição prona parece ser uma alternativa factível em circunstâncias excepcionais, nas quais a mobilização do paciente pode resultar em dano adicional, atrasar ou interromper o suporte avançado de vida (compressões torácicas precoces de alta qualidade e a desfibrilação) ou, ainda, incorrer em riscos ocupacionais à equipe de saúde.

Abstract Introduction: Postpartum hemorrhage is an obstetric emergency with high prevalence and significant morbidity and mortality, especially in areas with reduced access to specialized health services. Objective: To evaluate the effectiveness of intrauterine balloon tamponade in controlling postpartum hemorrhage, with the aim to reduce the need for emergency surgical interventions and decrease maternal mortality. Methods: A systematic review of randomized clinical trials, guided by the Cochrane Handbook for Systematic Reviews of Interventions and reported through the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Randomized clinical trials that evaluated the use of different types of balloons for intrauterine tamponade as a strategy for reducing or stopping postpartum hemorrhage compared to other interventions (pharmacological or surgical) were considered for inclusion. Results: Four studies evaluated 498 patients. In 80% of the reported cases, hemorrhage cessation was observed within a mean interval of 15 min after device insertion. The device permanence time was 24 h. No serious adverse events were reported. Due to clinical heterogeneity between studies, it was not possible to perform a quantitative synthesis. Conclusion: We did not find enough evidence to support the routine use of uterine tamponade devices as a protocol practice in the control of refractory postpartum hemorrhage. However, the use of these devices seems to be promising in cases where first line interventions fail and may play an important role in decreasing maternal morbidity and mortality and in uterine preservation.


Resumo Introdução: A hemorragia pós-parto trata-se de uma emergência obstétrica com elevada prevalência e morbimortalidade significativa, sobretudo em contextos de baixa acessibilidade a serviços especializados de saúde. Objetivo: Avaliar a efetividade do tamponamento por balão intrauterino no controle da hemorragia pós-parto, redução da necessidade de intervenções cirúrgicas de emergência e redução da mortalidade materna. Métodos: Revisão sistemática de ensaios clínicos randomizados, orientada pelo Cochrane Handbook for Systematic Reviews of Interventions e relatada através do Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Foram considerados como critérios de elegibilidade ensaios clínicos randomizados que avaliaram o uso de diferentes tipos de balão para tamponamento intrauterino enquanto estratégia para a redução ou cessação da hemorragia pós-parto quando comparados a outras intervenções (farmacológicas ou cirúrgicas). Resultados: Quatro estudos avaliaram 498 pacientes para os desfechos preconizados. Em 80% dos casos relatados observou-se a cessação da hemorragia em um intervalo médio de 15 minutos, após a inserção dos dispositivos. O tempo de permanência dos dispositivos foi de 24 horas. Não foram relatados eventos adversos graves. Devido à heterogeneidade clínica entre os estudos, não foi possível realizar síntese quantitativa. Conclusão: Os achados obtidos não fornecem evidências suficientes para sustentar a utilização rotineira dos dispositivos de tamponamento uterino enquanto prática protocolar no controle da hemorragia pós-parto refratária. A utilização destes dispositivos, no entanto, parece ser promissora diante da falha das intervenções de primeira linha, podendo desempenhar um importante papel em termos de redução de morbimortalidade materna e preservação uterina.

ABSTRACT Purpose: to recognize the effects of valproic acid (VPA), an epigenetic drug, on the bladder healing process, in rats. Method: twenty male Wistar rats were divided in two groups: experimental (A), treated with VPA (150mg/Kg/day), and control (B) with 0.9% sodium chloridrate. Healing was analyzed on the third and seventh days, evaluating the inflammatory reaction, collagen synthesis and angiogenesis. Results: inflammatory reaction on the third day was minimal and acute in both groups. On the seventh day, it was subacute in both groups, moderate intensity in group A and minimal in group B (p=0.0476). Collagen III intensity, marked by immunohistochemistry, was similar in both groups. Collagen I intensity on the third day was similar in both groups, but on the seventh day it was higher in experimental than control (p=0.0476). Collagen evaluation by picrosiriusred allowed to verify that the presence of collagen III was similar in both groups (p=0.3312) on the third day, and it was higher in control on the seventh day (p=0.0015). Collagen I showed similarity on the third day (p=0.3100), and it was higher in control on the seventh day (p=0.0015). Vessel marked with anti-SMA counting showed fewer vessels on the third (p=0.0034) and seventh day (p=0.0087) in experimental group. The lower intensity of angiogenesis was confirmed with anti-CD34, on the third day (p=0,0006) and on the seventh day (p=0,0072). Conclusion: VPA determined alterations in the bladder healing process, in rats, with lower collagen density and less angiogenic activity, but without compromising the integrity of the organ.


RESUMO Objetivo: reconhecer os efeitos do ácido valpróico (VPA), uma droga epigenética, no processo de cicatrização da bexiga, em ratos. Método: vinte ratos Wistar machos foram divididos em dois grupos: experimental (A), utilizando VPA (150mg/Kg/dia), e controle (B), tratados com cloreto de sódio 0,9% por gavagem. A cicatrização da bexiga foi analisada no terceiro e sétimo dia, estudando-se a reação inflamatória, síntese de colágeno, reepitelização e angiogênese. Resultados: a reação inflamatória no terceiro dia foi mínima e aguda em ambos os grupos. No sétimo dia, foi subaguda em ambos os grupos com intensidade moderada no grupo A e mínima no grupo B (p=0,0476). A intensidade do colágeno III, marcada pela imuno-histoquímica, foi semelhante nos dois grupos, nos dois tempos estudados. A intensidade de colágeno I no terceiro dia foi semelhante nos dois grupos, e maior no sétimo dia no grupo experimental (p=0,0476). A avaliação do colágeno pelo picrosiriusred mostrou que a presença de colágeno III foi semelhante em ambos os grupos (p=0,3312) no terceiro dia, e maior no controle no sétimo dia (p=0,0015). O colágeno I foi semelhante no terceiro dia (p=0,3100), e maior no controle no sétimo dia (p=0,0015). A contagem de vasos marcados pelo anti-SMA mostrou menos vasos no terceiro (p=0,0034) e sétimo dia (p=0,0087) no grupo experimental, confirmado pelo anti-CD34, no terceiro (p=00006) e no sétimo dia (p=0,0072). Conclusão: o VPA determinou alterações no processo de cicatrização da bexiga, em ratos, com menor densidade de colágeno e menor atividade angiogênica, mas sem comprometer a integridade do órgão.

ABSTRACT Purpose: To compare biological compatibility, hemostasis, and adhesion formation between oxidized regenerated cellulose and lyophilized hydrolyzed porcine collagen in liver trauma. Methods: Forty male Wistar rats constituted two groups: group A (oxidized cellulose) and group B (lyophilized hydrolyzed collagen). Standardized liver trauma was made, and the hemostatic agent was applied. Animals in subgroups A7 and B7 were submitted to euthanasia and relaparotomy after seven days, and in subgroups A14 and B14 after 14 days. Macroscopic and microscopic results were evaluated. Results: There was no fluid in the cavity in any of the animals, and adhesions were present in all of them. In the analysis after seven days, the adhesions were grades 3 or 4 and consisted of omentum, small intestine, and abdominal wall (p<0.05). In both groups, the mesh was surrounded by a capsule, which was not observed after 14 days. In the evaluation after 14 days, adhesions were grades 2 or 3 (p>0.05). The microscopic examination showed subacute and chronic reactions, in both groups and in both timepoints, with similar frequency. The intensity of fibrosis always presented positive scores. Microabscesses and xanthomatous macrophages were observed in both groups. Conclusions: There was no superiority of one agent over the other.

SUMMARY OBJECTIVE: The aim of our study was to investigate whether there has been a reduction in patient admission to a high-complexity cancer care center in Brasil during the COVID-19 pandemic, similar to what was reported in Europe. METHODS: We reviewed the cancer tracking database of the largest cancer center in southern Brasil and performed statistical tests to compare first-time appointments from the onset of the outbreak until the end of June to those of the equivalent period in 2019. RESULTS: We observed a dramatic reduction (-42%) in first-time appointments during the pandemic compared to the same period in the previous year (P <0.001). This reduction was observed among all medical specialties (P <0.001). CONCLUSION: The onset of COVID-19 was correlated with a reduction in admission to a high-complexity cancer care center in Brasil. Since a delay in diagnosis and treatment may influence prognosis, it is important that cancer centers and public health strategies reinforce care for non-COVID-19 patients to prevent potentially unnecessary deaths.

ABSTRACT Objective: To identify geographically the beneficiaries categorized as prone to Type 2 Diabetes Mellitus, using the recognition of patterns in a database of a health plan operator, through data mining. Method: The following steps were developed: the initial step, the information survey. Development, construction of the process of extraction, transformation, and loading of the database. Deployment, presentation of the geographical information through a georeferencing tool. Results: As a result, the mapping of Paraná according to its health care network and the concentration of Type 2 Diabetes Mellitus is presented, enabling the identification of cause-and-effect relationships. Conclusion: It is concluded that the analysis of georeferenced information, linked to health information obtained through the data mining technique, can be an excellent tool for the health management of a health plan operator, contributing to the decision-making process in Health.


RESUMEN Objetivo: Identificar geográficamente a los beneficiarios categorizados como propensos a la enfermedad Diabetes mellitus tipo 2, utilizándose el reconocimiento de patrones en una base de datos de cierta compañía de seguro médico por medio de la minería de datos. Método: Se desarrollaron las siguientes etapas: fase inicial, levantamiento de información. Desarrollo, construcción del proceso de extracción, transformación y carga en la base de datos. Implantación, presentación de la información geográfica mediante la herramienta de georreferenciación. Resultados: Se presenta el mapeo de Paraná (Brasil) con relación a su red asistencial y la concentración de Diabetes mellitus tipo 2, proporcionando la identificación de las relaciones de causa-efecto. Conclusión: Se concluyó que el análisis de las informaciones georreferenciadas, vinculadas a las informaciones de salud obtenidas por la técnica de minería de datos, puede ser una excelente herramienta en la gestión de salud de cierta compañía de seguro médico, lo que contribuye al apoyo a la toma de decisiones en salud.

Método: Desenvolveram-se as seguintes etapas: fase inicial, levantamento de informações. Desenvolvimento, construção do processo de extração, transformação e carga do banco de dados. Implantação, apresentação das informações geográficas por meio da ferramenta de georreferenciamento. Resultados: Como resultados, apresenta-se o mapeamento do Paraná em relação a sua rede assistencial e a concentração de Diabetes Mellitus Tipo 2, oportunizando a identificação de relações de causa-efeito. Conclusão: Conclui-se que a análise de informações georreferenciadas, vinculadas às informações de saúde obtidas por meio da técnica de mineração de dados, pode ser um excelente instrumento para a gestão da saúde de uma operadora de plano de saúde, contribuindo para o apoio à tomada de decisões em saúde.

RESUMO Objetivo: comparar a cicatrização, por segunda intenção, sob os efeitos da aplicação tópica de mel, óleo-resina de copaíba e um produto comercial (fibrinolisina, desoxirribonuclease e cloranfenicol) a um grupo controle, em ratos. Métodos: ressecção de pele, com 1cm de diâmetro, foi realizada no dorso de 40 ratos alocados em quatro grupos de dez animais. Todas as feridas foram limpas, diariamente, com 2ml de solução de NaCl 0,9%. O primeiro grupo (controle - GC) ficou restrito a tal procedimento. Nas feridas do segundo (GM), terceiro (GO) e quarto grupos (GF), após limpeza, aplicou-se, respectivamente, 1ml de mel, 1ml de óleo-resina de copaíba e 1ml de creme contendo fibrinolisina, desoxirribonuclease e cloranfenicol. Ocluíram-se as feridas com gaze estéril. Imediatamente após a incisão e nos dias três, sete e 14 do experimento, as feridas foram copiadas e, usando planimetria, analisou-se a contração. Após a eutanásia, a histologia foi utilizada para avaliação da reação inflamatória e do colágeno nas cicatrizes. Resultados: a redução da área da ferida do GM (p=0,003), GO (p=0,011) e GF (p=0,002) foram superiores ao do GC. A quantidade de colágeno tipo I presente no GM e no GO foi superior aos grupos GC e GF (p<0,05). Houve predominância do estágio inflamatório crônico no GM (p=0,004), GO (p<0,001) e GF (p=0,003) quando comparados ao GC. Conclusão: o uso tópico do mel e do óleo-resina de copaíba aumenta a contração da ferida, a presença de colágeno tipo I e acelera a cicatrização.


ABSTRACT Objective: to compare the healing by second intention under the effects of topical application of honey, copaíba oil-resin and a commercial product (fibrinolysin, deoxyribonuclease and chloramphenicol) with a control group in rats. Methods: we carried out a skin resection, 1cm in diameter, on the back of 40 rats allocated to four groups of ten animals. All wounds were cleaned daily with 2ml of 0.9% NaCl solution. The first group (control - GC) was restricted to such procedure. In the wounds of the second (GM), third (GO) and fourth groups (GF), after cleaning, we respectively applied 1ml of honey, 1ml of copaíba oil-resin and 1ml of cream containing fibrinolysin, deoxyribonuclease and chloramphenicol. The wounds were occluded with sterile gauze. Immediately after the incision and on days three, seven and 14 of the experiment, the wounds were copied and contraction was analyzed using planimetry. After euthanasia, we histologically evaluated the inflammatory reaction and collagen in the scars. Results: the reduction of the wound area of GM (p=0.003), GO (p=0.011) and GF (p=0.002) were higher than the GC. The amount of type-I collagen present in GM and GO was higher than in GC and GF groups (p<0.05). There was a predominance of chronic inflammatory stage in GM (p=0.004), GO (p<0.001) and GF (p=0.003) when compared with GC. Conclusion: the topical use of honey and copaíba oil-resin increases wound contraction, the presence of type-I collagen and accelerates healing.

Abstract Purpose: To evaluate the fibrosis induced by four different meshes: Marlex®, Parietex Composite®, Vicryl® and Ultrapro®. Methods: Histological cutouts of abdominal wall were analyzed with polarized light 28 days after the meshes implants and colorized by picrosirius to identify the intensity of collagen types I and III, and their maturation index. Results: When the four groups were compared, the total collagen area analyzed was bigger in groups A and D, with no difference between them. The collagen type I density was bigger in group A, with an average of 9.62 ± 1.0, and smaller in group C, with an average of 3.86 ± 0.59. The collagen type III density was similar in groups A, B and C, and bigger in group D. The collagen maturation index was different in each of the four groups, bigger in group A with 0.87, group B with 0.66, group D with 0.57 and group C with 0.33 (p = 0.0000). Conclusion: The most prominent fibrosis promotion in the given meshes was found on Marlex® (polypropylene mesh) and the Parietex Composite® (non-biodegradable polyester); the collagen maturation index was higher in the Marlex® mesh, followed by Ultrapro®, Parietex Composite® and Vicryl® meshes.

Summary Objective: To characterize the frequency of HER-2-positive breast cancer in Brazil. Method: In this prospective observational study, we first ascertained the HER-2 status of invasive breast cancer specimens by automated immunohistochemistry (IHC). For specimens classified as 2+ by IHC, we performed in situ hybridization (ISH). Results: From February, 2011 to December, 2012, 1,495 breast specimens were registered, and 1,310 samples collected at 24 centers were analyzed. Median patient age was 54 years, and the majority of samples were obtained from segmental (46.9%) or radical mastectomy (34.4%). The predominant histological type was invasive breast carcinoma of no special type (85%), 64.3% had tubule formation (grade 3), and estrogen/progesterone receptors (ER/PR) were positive in 77.4/67.8% of the specimens analyzed, respectively. Using IHC, we found a negative HER-2 status (0 or 1+) in 72.2% of specimens, and 3+ in 18.5%; the 9.3% scored as 2+ were further analyzed by ISH, of which 15.7% were positive (thus, 20.0% of samples were HER-2- -positive by either method). We found no association between HER-2 scores and menopausal status or histological type. Tumors classified as 3+ came from younger patients, and had higher histological grade and less frequent expression of ER/PR. In the North region of Brazil, 34.7% of samples were 3+, with lower frequencies in the other four regions of the country. Conclusion: Our findings provide estimates for the frequency of HER-2 positivity in Brazil and raise the hypothesis that biological differences may underlie the different distribution of breast-cancer phenotypes among different Brazilian regions.


Resumo Objetivo: Estimar a frequência de câncer de mama positivo para HER-2 no Brasil. Método: Neste estudo observacional e prospectivo, verificamos o escore de HER-2 de espécimes de câncer de mama invasivo por imuno-histoquímica automatizada (IHQ). Para amostras classificadas como 2+ por IHQ, fizemos hibridização in situ (HIS). Resultados: De fevereiro de 2011 a dezembro de 2012, 1.495 espécimes de mama foram registrados, e 1.310 amostras coletadas por 24 centros foram analisadas. A idade mediana das pacientes foi 54 anos, e a maioria das amostras foram obtidas a partir de mastectomia segmentar (46,9%) ou radical (34,4%). O tipo histológico predominante foi o carcinoma invasivo da mama, sem tipo especial (85%); 64,3% tinham formação de túbulos (grau 3); e os receptores de estrógeno (RE)/progesterona (RP) foram positivos em 77,4%/67,8% das amostras analisadas. Por IHQ, encontramos HER-2 negativo (0 ou 1+) em 72,2% das amostras, e 3+ em 18,5%; os 9,3% de casos classificados como 2+ foram analisados por HIS, e 15,7% deles foram positivos (assim, 20,0% das amostras foram positivas para HER-2 por qualquer método). Não encontramos associação entre escores de HER-2 e estado menopausal ou tipo histológico. Tumores classificados como 3+ vieram de pacientes mais jovens, tinham maior grau histológico e foi menos frequente a expressão de RE/RP. Na região Norte do Brasil, 34,7% das amostras foram 3+, com frequências mais baixas nas outras quatro regiões do país. Conclusão: Nossos resultados permitem estimar a frequência de positividade do HER-2 no Brasil, gerando a hipótese de que pode haver diferenças biológicas subjacentes à distribuição dos fenótipos de câncer de mama entre as diferentes regiões brasileiras.

ABSTRACT

ABSTRACT PURPOSE: To compare the two lines suture (total and seromuscular) after partial gastrectomy in normal and overweight rats. METHODS: Forty Wistar rats were distributed in two groups. Group A received normal diet; group B, normal diet and supplementation with saccharose in the water. When group B progressed to a statistically greater weight than the animals of group A, the experiment (sleeve-like gastrectomy) was conducted with gastrorraphy in two sutures lines (total and seromuscular).The animals were distributed into two subgroups of 10. A1 and A2 subgroups were sacrificed at 7 and 14 days postoperatively as well as B1 and B2. Mortality, morbidity, complications attributed to the gastric suture, biochemical dosages, Lee index, macroscopy, weight of retroperitoneal and gonadal fat, optical microscopy with hematoxylin-eosin and picrosirius-red, were the evaluation parameters. RESULTS: The overweight group achieved statistically greater weight after 16 weeks in induced obesity; there was no mortality or complications with clinical consequences attributable to morbidity. The overweight group had statistically greater weight of gonadal and retroperitoneal fat. The difference was observed in urea, albumin, total cholesterol and indirect bilirubin. CONCLUSION: There was no outcome difference between the overweight and non-overweight group in two suture lines in gastrorrhaphy after sleeve-like gastrectomy.

ABSTRACT

Objetivo: Este artigo apresenta um sistema inteligente baseado em regras de produção para apoiar o processo de auditoria em contas médicas. Método: Foram descobertas regras sobre procedimentos cirúrgicos, a partir de base de dados, posteriormente incorporadas, na base de conhecimento, no formato SE-ENTÃO. Possibilitando a seleção de contas médicas que poderão ou não ser submetidas à auditoria. O conjunto de regras foi avaliado e validado por especialistas em auditoria de contas médicas. Resultados: Foram descobertas inicialmente 199 regras e que após análise e validação dos especialistas resultaram num conjunto de 26 regras que integraram a base de conhecimento do sistema, que implementado selecionou de forma autônoma as contas médicas a serem auditadas, permitindo inclusive a retro avaliação das regras. Conclusão: O sistema inteligente utilizado no processo de indicação de contas para auditoria, otimizou o processo, reduziu a frequência de auditorias desnecessárias e melhorou a qualidade da auditoria.


Objective: This paper show an intelligent system based on production rules to support the audit process in medical bills. Method: Surgical procedures rules were discovered from a database, subsequently incorporated in the knowledge base, in the IF-THEN format, allowing the selection of medical bills that may or may not be subject to audit. The rules set was assessed and validated by experts in medical bills auditing. Results: Initially 199 rules were discovered and after analysis and validation by experts resulted in a set of 26 rules that integrated the system knowledge base, which implemented selected autonomously medical bills to be audited, including allowing the retro evaluation the rules. Conclusion: The intelligent system used in the selection of medical bills for auditing, optimized the process, reduced the frequency of unnecessary audits and improved audit quality.


Objetivo: Este artículo presenta un sistema inteligente basado en reglas de producción para apoyar el proceso de auditoría en cuentas médicas. Método: Fueron descubiertas reglas sobre procedimientos quirúrgicos, partiendo de base de datos, posteriormente incorporadas, en la base de conocimiento, con formulario SI-ENTONCES. Posibilitando la selección de cuentas médicas que podrán o no ser sometidas a la auditoría. Las reglas fueron evaluadas y validadas por especialistas en auditoría de cuentas médicas. Resultados: Fueron descubiertas inicialmente 199 reglas que después del análisis y validación de los especialistas resultaron en un conjunto de 26 que integraron la base de conocimiento del sistema, implementado seleccionó de forma autónoma las cuentas médicas a ser auditadas, permitiendo incluso la retro evaluación de las reglas. Conclusión: El sistema inteligente utilizado en el proceso de indicación de cuentas para auditoría, optimizó el proceso, redujo la frecuencia de auditorías innecesarias y mejoró la cualidad de la auditoría.

ABSTRACT PURPOSE: To compare fibroplasia and the resistance of the abdominal wall when polypropylene meshes and polypropylene/poliglecaprone are used. METHODS: Seventy-seven male Wistar rats were divided into three groups: Control Group (for resistance); Group E (polypropylene mesh); and Group U (polypropylene/poliglecaprone mesh). The animals in Groups E and U had a standard muscular and aponeurotic defect, with integral peritoneum, and correction with the mesh. Measurements were taken 4, 7, 14, 28 and 56 days after surgery. The resistance, and collagen density were studied. RESULTS: Resistance on the 56th day was similar in both meshes. The gain in resistance described an ascending curve for the polypropylene mesh and was irregular in the case of the polypropylene/poliglecaprone. Fibroplasia showed a gain in type I and type III collagen in both groups (p<0.001). Collagen III stabilized in the 14th day and collagen I continued to ascend. CONCLUSIONS: The gain in resistance of the polypropylene mesh is regular and ascending, whereas the polypropylene/poliglecaprone is not regular. The final resistance of both meshes is similar; the collagen density increases over time, and show the same inflammatory potential.

PURPOSE: To compare the inflammatory reaction and the growing resistance of the abdominal wall with the use of poliglecaprone meshes and polypropylene meshes associated with poliglecaprone in the correction of abdominal defects. METHODS: Seventy-seven Wistar rats were divided into three groups: CG (non-operated animals: EG (polypropylene mesh) and UG (polypropylene and poliglecaprone mesh). A muscular and aponeurotic defect was formed and treated according to the group. Evaluations were made after 4, 7, 14, 28 and 56 days. The resistance and inflammatory pattern were studied. RESULTS: There was a gradual and significant gain in resistance, regularly in the EG and irregularly in the UG, which was lower on the 14th day (p=0.008). The inflammatory reaction was acute and more intense in the UG on the fourth day. At all other times, the inflammatory pattern was acute to chronic, similar in both groups, with minimum intensity on the 56th day. CONCLUSION: The greater resistance offered by the polypropylene mesh was regular and ascending, stabilizing on the 28th day, while that of the polypropylene/poliglecaprone was not even. In the end, the resistances were similar. The inflammatory response was greater in the UG on the fourth day and similar at all other times. .

INTRODUCTION: Recent studies indicate a role of human epidermal growth factor receptor type 2 (HER2) in the development of numerous types of human cancer, including gastric cancer, and its overexpression correlates with poor prognosis and increased aggressiveness of this neoplasm. OBJECTIVES: Identify and evaluate the immunohistochemical expression of HER2 in gastric cancer from biopsies, surgical resection specimens, lymph node metastasis and distant metastasis, and evaluate their correlation with currently known clinical and histopathological prognostic factors. METHODS: Samples from 118 patients of both sexes and all age groups diagnosed with gastric cancer were analyzed. Immunohistochemistry (IHQ) was performed using the HER2 antibody, and its evaluation was made according to the modified gastric cancer testing protocol, taking into account incomplete basolateral staining or only lateral staining. RESULTS: HER2 expression did not correlate with any histological prognostic factors or clinical outcome, except for the N stage. By comparing the HER2 expression in biopsies, surgical specimen, lymph node and metastasis, 88.1% were in agreement. CONCLUSION: As anti-HER2 therapies are becoming the standard of care in gastric cancer, currently available data indicate that IHQ should be used as the screening test, and the pathologist has an important role to ensure an accurate testing of HER2 status in these tumors. In addition, the HER2 status can be tested using the available samples from biopsies, surgical specimens, nodal and extranodal metastatic disease.


INTRODUÇÃO: Estudos recentes indicam o papel do human epidermal growth factor receptor type 2 (HER2) no desenvolvimento de vários tipos de câncer, incluindo o gástrico. Sua superexpressão correlaciona-se com pior prognóstico e maior agressividade dessa neoplasia. OBJETIVOS: Identificar e avaliar a expressão imuno-histoquímica de HER2 no câncer gástrico de biópsias, espécimes de ressecção cirúrgica, metástase linfonodal e metástase a distância, bem como analisar sua correlação com fatores prognósticos clínicos e histopatológicos conhecidos atualmente. MÉTODOS: Amostras de 118 pacientes de ambos os sexos e todas as faixas etárias diagnosticados com câncer gástrico foram analisadas. A imuno-histoquímica foi realizada utilizando o anticorpo HER2, e a sua avaliação foi feita de acordo com o protocolo do teste modificado do câncer gástrico, levando em conta a coloração basolateral ou lateral incompleta. RESULTADOS: A expressão do HER2 não apresentou correlação com quaisquer fatores histológicos e prognósticos ou evolução clínica, com exceção do estádio N. Ao comparar a expressão HER2 nas biópsias de peças cirúrgicas, linfonodos e metástases, observou-se que 88,1% dos espécimes apresentaram concordância. CONCLUSÃO: Como as terapias anti-HER2 estão se tornando o padrão para o tratamento do câncer gástrico, os dados atualmente disponíveis indicam que a imuno-histoquímica deve ser usada como teste de rastreamento. O patologista tem um papel importante para determinar a expressão do marcador nesses tumores. Ademais, o status do HER2 pode ser testado usando as amostras disponíveis de biópsias, peças cirúrgicas e doença metastática nodal e extranodal.

ABSTRACT

Strumal carcinoid tumor is an uncommon type of ovarian teratoma containing thyroid and carcinoid tissue. We report the case of a 49 year-old patient with menstrual irregularity. The ultrasonographic imaging revealed a mass in the left ovary. The histopathological assessment showed a papillary thyroid carcinoma in a strumal carcinoid tumor.


Carcinoide strumal é uma forma incomum de teratoma ovariano que contém tecido tireoidiano e carcinoide. Relatamos o caso de uma paciente de 49 anos de idade, cuja investigação por irregularidade menstrual revelou massa em ovário esquerdo à ultrassonografia. A avaliação histopatológica da massa apresentou carcinoma papilífero de tecido tireoideano em carcinoide strumal.

PURPOSE: To determine the impact of hypertension in liver regeneration, in rats by examining gain in liver mass and the replication of hepatocytes and stellate cells. METHODS: Forty Wistar rats were allocated into two groups of twenty, the control and experiment group. The experiment group animals were submitted to induction of renovascular hypertension. A week later, all the animals underwent a partial hepatectomy. Measurements were taken after 24 hours and seven days, when ten animals in each group were euthanized. Thus, four subgroups were obtained. The livers were excised and sent for histopathological analysis. RESULTS: The control group had a greater gain in liver mass than the experiment group seven days after partial hepatectomy (p=0.0051). The difference in the activate stellate cell count was not statistically significant following analysis after both 24 hours and seven days (p=1.0). A higher number of dividing hepatocytes was observed in the control group seven days after partial hepatectomy (p=0.0014). CONCLUSION: In rats, hypertension had no direct influence on stellate cell replication, but led to a delay in liver mass gain and were shown to be a reduction factor on hepatocyte replication seven7 days after partial hepatectomy.


OBJETIVO: Determinar o impacto da hipertensão arterial sistêmica na regeneração hepática, em ratos, através da análise do ganho de massa hepática e da replicação dos hepatócitos e das células estreladas. MÉTODOS: Alocaram-se 40 ratos Wistar em dois grupos de 20 animais, os grupos controle e experimento. Os do grupo experimento submeteram-se a indução da hipertensão renovascular. Uma semana após, realizou-se hepatectomia parcial em todos os animais. Colheram-se os dados com 24 horas e sete dias, quando dez animais de cada grupo submeteram-se a eutanásia. Assim, obtiveram-se quatro subgrupos. Os fígados foram retirados e enviados para análise histopatológica. RESULTADOS: O grupo controle apresentou maior ganho de massa hepática do que o grupo experimento sete dias após a hepatectomia parcial (p=0,0051). A diferença na contagem das células estreladas ativadas não foi estatisticamente significante nas análises de 24 horas e de sete dias (p=1,0). Um maior número de hepatócitos em divisão foi observado no grupo controle, sete dias após a hepatectomia parcial (p=0,0014). CONCLUSÃO: Em ratos, a hipertensão não teve influência direta sobre a replicação de células estreladas, mas levou ao atraso no ganho de massa hepática e mostrou ser um fator de redução na replicação de hepatócitos sete dias após a hepatectomia parcial.

RACIONAL: O tabagismo na gestação acarreta prejuízos para o crescimento intra-uterino da criança. Assim como na gestação, as suas consequências sobre a lactação e a criança amamentada constituem somatório de efeitos prejudiciais ao desenvolvimento do lactente. OBJETIVO: Avaliar a influência da nicotina na proliferação de miofibroblastos e de vasos sanguíneos no tecido cicatricial da parede abdominal de ratos lactentes nascidos e amamentados por ratas que receberam nicotina durante a gestação e lactação. MÉTODO: Ratas Wistar prenhas foram divididas aleatoriamente em dois grupos; um grupo recebeu nicotina (GN) na dose diária de 2 mg/kg de peso durante a gestação e lactação; o grupo controle (GC) recebeu solução fisiológica a 0,9 por cento, no mesmo período. Foram utilizados 60 ratos lactentes Wistar, divididos igualmente em dois grupos. Após o término do período de lactação (21 dias), todos foram submetidos à laparotomia transversa e ao fechamento da camada músculo-aponeurótica. Cada grupo foi dividido em dois subgrupos, com 15 animais cada, de acordo com o intervalo de tempo utilizado para a avaliação. As aferições foram realizadas no 7.º dia (subgrupos C7 e N7) e no 21.º dia (subgrupos C21 e N21) de pós-operatório. Foram retirados os retalhos da parede abdominal contendo a linha de sutura, os quais foram estudados pelo método imunoistoquímico para identificação de miofibroblastos e de vasos sanguíneos. A estatística foi baseada no modelo ANOVA, ao nível de significância de 5 por cento. RESULTADOS: No subgrupo N7 houve redução do número de miofibroblastos (9,93 ± 3,06 vs. 21,87 ± 9,07, p=0,007) e do número de vasos sanguíneos (8,33 ± 4,43 vs. 13,4 ± 5,33, p=0,031) em comparação ao C7. No subgrupo N21, não houve diferença significativa do número de miofibroblastos (7,47 ± 3,96 vs. 12,00 ± 7,21, p=0,121) e do número de vasos sanguíneos (9,47 ± 2,42 vs. 12,93 ± 4,35, p=0,090) em comparação ao C21. BACKGROUND: Smoking during pregnancy is detrimental to the intrauterine growth of the child. As in pregnancy, its effects on lactation and breast-fed child is the sum of adverse infant development. AIM: To analyze the effects of nicotine on the proliferation of myofibroblasts and of blood vessels, on the abdominal scar of suckling rats born of mothers that received nicotine, during pregnancy and lactation. METHODS: Pregnant Wistar rats were randomly divided into two groups, the nicotine group (NG), in which nicotine was administered subcutaneous at a dose of 2 mg/kg/day, during pregnancy and lactation, and the control group (CG), that received subcutaneous isovolumetric saline solution at 0,9 percent, in the same period of time. Sixty baby rats, divided into two groups, were weaned at 21 days and a transverse laparotomy was performed. Each group was divided into two subgroups, each one containing 15 baby rats, in accordance with the date when the surgical scar was analyzed: on the 7th post-operative day, control subgroup C7 and nicotine subgroup N7, or on the 21st post-operative day, control subgroup C21 and nicotine subgroup N21. The surgical scar in the healing area was evaluated by immunohistochemistry study for identification of myofibroblasts and of blood vessels. The statistical analysis was based on the Anova model, at a significance level of 5 percent. RESULTS: The subgroup N7 presented lower number of myofibroblasts (9,93 ± 3,06 vs. 21,87 ± 9,07, p=0,007) and lower number of blood vessels (8,33 ± 4,43 vs. 13,4 ± 5,33, p=0,031) when compared to C7. The subgroup N21 presented no significant difference on the number of myofibroblasts (7,47 ± 3,96 vs. 12,00 ± 7,21, p=0,121) and on the number of blood vessels (9,47 ± 2,42 vs. 12,93 ± 4,35, p=0,090) when compared to C21. RACIONAL: As medicações mais frequentemente prescritas e usadas de forma crônica por pacientes com queixas dispépticas pertencem ao grupo dos inibidores de bomba de prótons cujo principal representante é o omeprazol, utilizado clinicamente a cerca de 20 anos. Estudos recentes têm postulado a relação entre o uso crônico desta medicação e alterações proliferativas macroscópicas e microscópicas da mucosa do fundo e corpo gástrico, principalmente os pólipos de glândulas fúndicas. OBJETIVO: Analisar a frequência e o tipo de alterações proliferativas gástricas em usuários crônicos de inibidores de bomba de prótons e sua associação com idade, dose utilizada, tempo de uso, sintomatologia e infecção pelo Helicobacter pylori. MÉTODO: Estudo transversal de prevalência realizado no Serviço de Endoscopia Digestiva do Hospital de Clínicas da Universidade Federal do Paraná. Aplicado questionário com informações quanto ao uso destas drogas e realizada endoscopia digestiva alta em sujeitos em uso contínuo desta classe de medicamentos há pelo menos 12 meses. Realizadas biópsias de fundo, corpo e antro gástrico em todos os pacientes. Pólipos eram retirados ao serem identificados, para análise histopatológica. Realizada pesquisa do Helicobacter pylori por teste de urease e análise histopatológica. RESULTADOS: Vinte e dois indivíduos foram incluídos sendo que seis (27,3 por cento) apresentaram alterações proliferativas da mucosa oxíntica. Destes, dois (9,1 por cento) apresentavam pólipos fúndicos esporádicos e quatro apresentavam exclusivamente alteração proliferativa microscópica como formação cística glandular. Houve significância estatística (p<0,05) na associação entre idade > 60 anos e estas alterações. Não houve associação entre dose de uso, tempo de uso, sintomatologia e infecção pelo Helicobacter pylori. BACKGROUND: The medications most often prescribed and used chronically by patients with dyspeptic symptoms belong to the group of inhibitors of proton pump whose main representative is omeprazole, used clinically about 20 years. Recent studies have postulated the relationship between chronic use of this medication and macroscopic and microscopic proliferative changes in mucosal of gastric body, especially the fundic gland polyps. AIM: To analyze the frequency and type of gastric proliferative changes in chronic users of proton pump inhibitors proton and its association with age, dose, time of use, symptoms and Helicobacter pylori infection. METHODS: A survey of prevalence was conducted in Digestive Endoscopy Service, Hospital de Clínicas, Federal University of Paraná, using questionnaire referring the use of these drugs and performed in subjects using this class of drugs during at least 12 months prior to the endoscopy. Gastric biopsies of fundus, body and antrum were performed in all patients. Polyps were removed when present for histopathological analysis. Research on Helicobacter pylori was done by urease test and histopathology. RESULTS: Twenty-two individuals were included of which six (27.3 percent) had proliferative changes of the mucosa. Of these, two (9.1 percent) had sporadic fundic polyps and four only microscopic proliferative change as cystic glandular formation. Statistic significant (p <0.05) association between age > 60 years and these changes was found. There was no association between dose of use, time of use, symptoms and Helicobacter pylori infection. CONCLUSIONS: It was observed the existence of proliferative mucosa changes in individuals with chronic use of inhibitors of proton pump. In this group, the association between age and proliferative changes was significant.
